Voluntary Term Life

Principal – 100% employee paid (premiums based on earnings, amount of coverage selected, and age.)

Disability ST & LT

Principal – 100% employee paid (premiums based on earnings, amount of coverage selected, and
age.) Short Term is 60% of weekly earnings max of $1,500 per week and starts 8th day of leave, maximum benefit
duration 12 weeks. Long Term is 60% of earnings max of $10,000 per month; benefits begin on 91st day of leave.

FSA –Flexible Spending Accts

You can set aside pre-tax dollars for out-of-pocket expense incurred for either health
care of dependent day care. Maximum yearly contribution is $3,050.00 and unused balances over $610 will be lost at the
end of the plan year. Can be used with Traditional PPO and High Deductible Plans. (re enrollments each April)

HSA- Health Saving Accts

You can set aside pre-tax-dollars for out-of-pocket health care expenses; funds roll over from
year to year. Company will match up to $500.00 per year. Maximum Yearly contributions Single Coverage $3,850, Family
Coverage $7,750. (Must be enrolled in the High Deductible Plan)


Eligible to participate after 6 months of service and 21 years of age Enrollments are at the first of each calendar
quarter. The company matches 50% of contributions up to a max of $2,500/ annually. (If you put in $5,000 the company
will put in $2,500.)


Eligible for paid holidays once completed 30 days of service 7 holidays

VACATION (Salary Personnel)

After 3 months of service earn 1 week. After initial hire, time is awarded annually at the
start of each year based on the following tenure schedule: 1st year – one week, 1 to 10 years – two weeks, 10 to 20
years – 3 weeks, over 20 years – 4 weeks. Time awarded must be used by the end of the calendar year. There is no

VACATION (Hourly Personnel)

Eligible for accrual after 3 months of service. See chart below for accrual rates by service year.

1-9 years 3.08 per pay period= 80 hours annually (2 weeks)
10-19 years 4.62 per pay period= 120 hours annually (3 weeks)
20+ years 6.16 per pay period= 160 hours annually (4 weeks)
Carryover is a maximum of 120 hours (3 weeks)


Earn 3 days after 1 year of service Allotted annually on anniversary date for hourly employees only.


Employees must be accident free and attend all required safety trainings/meetings for the
current year. Employees will be awarded one paid day off to be used in the following year as scheduled and approved by
the supervisor.


Employees enrolling in outside education courses that are relevant to our business and
who have prior approval of their department managers and the Human Resources Director will be reimbursed for the
cost of tuition upon satisfactorily completing the course (B or better). Approval of any education assistance is at the sole
discretion of the Company and may be based upon the course and its usefulness to either your current position or
another within the company as well as other factors including the cost of the program. The reimbursement is limited to
a maximum of $2,000 per 12-month period and $8,000 lifetime per employee.


To encourage employees to refer qualified candidates for position openings, a $200 referral fee
will be paid to any non-hiring employee who refers a candidate who is hired and successfully completes 90 days of


For those who require corrective lenses, the company will pay up to
$150 every two years for prescription safety glasses. All safety glasses must have side shield protection. To be
reimbursed, a receipt must be submitted.


The company will provide up to $100 per calendar year for the purchase of safety toe


Employees who have an annual physical (wellness/preventative) performed by a doctor will be
awarded an additional day (one per calendar year) of vacation. To receive your extra day of vacation, you must complete
Part 1 of the Wellness Exam Form and have the physician complete Part 2 stating they have performed the exam. Once
completed, the form should be turned in to HR. The vacation day will be awarded in the next pay period following
verification of exam.


Pay dates are bi weekly on Thursdays which cover the previous two weeks. Pay periods run from Sunday –
Saturday. Overtime (for applicable positions) is paid for any hours to be paid that are over 40 in a week (we do include
sick/vac time towards the over 40 calculations in determining overtime)
